5.15 Case note audit of missed SGA

  • The audit tool can be adapted to mirror DHB guidelines on fetal growth surveillance.
  • A minimum of 30 cases each 6 months is recommended in order to identify trends.
  • Action plans are produced as a result of each audit,  to ensure all trends are reviewed and actioned where issues are identified.
  • Trends may include;
    • Training requirements for clinicians.
    • Guidelines  and scanning resources.
    • Service provision and  to inform commissioning.
