5.15 Case note audit of missed SGA

The audit tool can be adapted to mirror DHB guidelines on fetal growth surveillance.
A minimum of 30 cases each 6 months is recommended in order to identify trends.
Action plans are produced as a result of each audit, to ensure all trends are reviewed and actioned where issues are identified.
Trends may include;
- Training requirements for clinicians.
- Guidelines and scanning resources.
- Service provision and to inform commissioning.
